What Exactly is Bariatric Surgery?  
Bariatric surgery stands as a beacon of hope for individuals who have battled severe obesity and the health issues it spawns. By surgically altering the digestive system, it offers a powerful method for significant weight loss. This section elucidates the essence of bariatric surgery, its historical evolution, and how it has become a sophisticated, safe, and effective tool in the fight against obesity.

The origins of bariatric surgery can be traced back to the 1950s and 1960s, when the first procedures were developed to treat severe obesity. Over the decades, new procedures have been added and older ones refined. Some procedures have disappeared over time, and others have soared in popularity. The evolution of bariatric surgery is a testament to the medical community's commitment to finding effective treatments for obesity. Types of Surgery

Gastric Sleeve
Over the past ten years, the Gastric Sleeve procedure has risen to become the most favored method for weight loss. This technique is carried out laparoscopically, involving small incisions in the skin and the use of a camera for guidance. During the surgery, a part of the stomach is surgically removed, resulting in a newly shaped stomach about the size and shape of a banana. This reduction in stomach size limits the quantity of food needed to achieve satiety. Furthermore, we target the section of the stomach responsible for producing Ghrelin, the hunger hormone. With the reduction of Ghrelin, patients typically experience a decrease in hunger sensations. Consequently, a significant reduction in calorie intake leads to substantial weight loss.

Patients have expressed high levels of satisfaction with the Gastric Sleeve procedure. They find that their reduced-size stomach still functions quite normally, allowing them to enjoy their favorite foods and dine at their preferred restaurants, but with a considerably smaller capacity. Unlike other procedures that reroute the intestines, the Gastric Sleeve has minimal impact on the absorption of nutrients, making it an effective and preferable option. When performed on suitable candidates, this operation can yield remarkable outcomes.  
Gastric Sleeve
The Gastric Sleeve has become the most popular weight loss procedure over the last decade. 

It is performed laparoscopically, using tiny skin incisions & a portion of the stomach is removed, leaving behind a smaller stomach about the size of a banana. This smaller stomach means you eat a smaller amount of food to feel full. Additionally, the part of the stomach that produces the hunger hormone Ghrelin is removed. This hormone plays a role in causing the feeling of hunger. After the Sleeve, as levels of Ghrelin decrease, and feeling of hunger decreases. Significantly less calories consumed leads to massive weight loss.

We find that patients are extremely satisfied with this procedure. They go about their daily lives with a stomach that still feels pretty normal, although significantly smaller. They still enjoy their favorite foods, and go to their favorite restaurants. Since their is no re-routing of intestines, there is minimal malabsorption of nutrients. It's a great operation, and when performed on the right patients, and can give fantastic results.   
Gastric Bypass
Gastric Bypass is a procedure where a portion of the stomach is sectioned off, creating a small stomach pouch about the size of an egg. This stomach pouch is directly attached to your intestine so food bypasses the larger stomach. This small stomach pouch causes the feeling of fullness after just a few bites of food, and eating a lot less is where a majority of your weight loss comes from. Additionally, 3-5 feet of small intestine is bypassed, which helps with greater weight loss. 

This procedure is performed laparoscopically using tiny skin incisions in the abdomen. This offers smaller scars and a quicker recovery time. This procedure is fully reversible, if needed. There are some differences between the Sleeve and the Bypass. This works better for treatment of acid reflux (GERD), delivers more weight loss, and patients have a higher long term success compared to the gastric sleeve or the gastric-band.

FAQS

WHICH PROCEDURE IS BEST FOR ME?
To help choose the right procedure, several factors are considered, including your age, BMI, health conditions, previous surgeries, lifestyle, weight loss goals, and the risks and benefits of each procedure. All the options are discussed with Dr. Mehta during the consultation. 

HOW MUCH TIME OFF IS NEEDED?
For the Lap-Band, people go home the same day, as for the Gastric Sleeve and Bypass, nearly all of our patients go home after 1 overnight stay at the hospital. For the lap-band, most people can return to work in 3 days. For a Gastric sleeve or gastric bypass, at least 1 week off minimum is needed before returning to office type work.

HOW SOON CAN I HAVE SURGERY?
The process starts with an initial consult with Dr. Mehta, in person or virtual. We then initiate the surgery approval process with your health insurance. Our office does the insurance work, and has a very high approval rate. Once approved by insurance, you can have surgery within 2 weeks.  Self-pay patients can be placed on fast-track, and can often surgery in as little as 10 days.

HOW MUCH WEIGHT WILL I LOSE?
Overall weight loss depends on many factors. Since you are unique, and so is your weight loss journey, diet & lifestyle, how much weight you will lose can only be estimated after putting your entire picture together regarding your age, height, weight, medical illnesses, and lifestyle. Smaller patients can lose around 50 lbs. but larger ones can lose up to 180 lbs. or more. It depends on which procedure we are discussing. Results vary based on many factors such as age, sex, and conforming with proper post-op nutrition guidelines.

Alternative Options
What about Gastric Balloons?
Inserting a balloon or balloons in the stomach and later removing them months later cannot and does not have any long lasting weight loss. Patients may lose 10% of their body weight which almost always is regained once the balloons are removed. Secondly, balloons are expensive and not covered by insurance. There are 1 to 2 endoscopic procedures involved to insert and later remove the balloons.

Endoscopic Sleeve Gastrectomy (ESG)?
ESG is a minimally invasive procedure performed using an endoscope, a flexible tube with a camera and an endoscopic suturing device attached, which is inserted through the mouth and into the stomach. The stomach's size is reduced by creating a series of sutures to cinch in large portions of the stomach, without removing any stomach tissue. The stomach capacity decreases and patients eat less and lose weight. The main problems with this procedure is that the sutures are usually grabbing the inner lining of the stomach, thus, many of the sutures do not hold in place long term. Secondly, the stomach isn't nearly as small as would be with a gastric sleeve. thus the weight loss is about 10% of your body weight compared to 70% for the Gastric Sleeve, and even more for a Gastric bypass.

SELF PAY- Self-pay option is available for each procedure includes surgery fee, hospital OR fee, anesthesia fee, one overnight stay at the hospital (if required), and 1 year of follow-ups (in-person or using tele-health) including dietary counseling. Immediate appointments are available and you can have the surgery within weeks. 

FAST-TRACK - We can fast-track these procedures, especially for self-pay, since the insurance approval timing is eliminated, and you can likely have the procedure within 10 days.
